Take the Health Counts Survey
On behalf of the Health Planning Council of Northeast Florida and the Jacksonville Nonprofit Hospital Partnership, we would like to thank you for your participation in the Key Stakeholder Interviews. Your dedication to your county, your knowledge, and expertise will be a valuable contribution to this effort, and further guide the direction of the Community Health Needs Assessment.
In continuation with our assessment, we have created the Community Health Needs Assessment Survey, which focuses on health equity and disparities. This survey will be used to capture the challenges and experiences of the residents in Baker, Clay, Duval, Nassau, and St. Johns Counties. As a valued Key Stakeholder with extensive outreach and community presence, we would like to solicit your participation in reaching residents in your county. I have attached a copy of the survey flyer to be included on your organization’s website and social medial handles. The survey and flyer will also be available in Spanish. If your organization would like paper copies of the survey or access to the Spanish version of the link or graphic, please notify our office.
